The following forms must be completed and submitted as soon as possible after you are accepted as a delegate. Send the original plus two copies of the completed forms to International Partners, 15437 Tindlay St., Silver Spring, Maryland 20905.
- Participant Consent and Release
- Parent(s) or Guardian Consent and Release
- Physician’s Report
- Confidential Medical History
- Behavior Policies and Guidelines Agreement
- Special Delegation Inquiry Form (Groups only)
- Photo page of your passport
- Travel authorization letter for students less than 16 years of age
